What is the Assistive Devices Program?

The Assistive Devices Program (ADP) is a program provided by the Ontario Ministry of Health and Long-Term Care. The objective of ADP is to financially assist Ontario residents with long-term physical disabilities to obtain basic, competitively priced, personalized assistive devices appropriate for the individual's needs and essential for independent living. Ostomy supplies falls under this description.

Who can apply?

Any resident of Ontario with a valid Health Card and who has a permanent colostomy, ileostomy, urostomy, ileal conduit or continent pouch/reservoir. ADP does not pay for supplies for persons with a temporary ostomy.

ADP does not pay for supplies for persons who reside in chronic care hospitals, Ministry of Community, Family and Children's Services residential facilities, or people in Ministry of Health and Long-Term Care facilities.

ADP does not pay for supplies available under the Workplace Safety and Insurance Board (WSIB) or to Group 'A' veterans for their pensioned conditions.  [Top]

How do I apply?

You begin by obtaining an Ostomy Grant Applicant Information form. You can get this form from your doctor or by calling one of these Ministry of Health and Long-Term Care phone numbers.

Follow the instructions on the form. You and your doctor must both complete certain sections of the form. Your doctor must certify that you have a permanent colostomy, ileostomy, urostomy, ileal conduit or continent pouch/reservoir. You must sign and date the bottom of the form. Send the completed form to:

Amount of the Grant

If you have an ostomy, you are eligible to receive $600 per year.

If you have two ostomies, for example, a colostomy and a urostomy, you are eligible to receive $1,200 per year.

If you are receiving social assistance benefits under Ontario Works, Ontario Disability Support Program, or Assistance to Children with Severe Disabilities, you may be eligible to receive $800 per ostomy.

Residents of long-term care facilities are eligible to receive $800 per ostomy.  [Top]

Payment of the Grant

You will receive two cheques in equal amounts. The first cheque will arrive approximately six weeks after the Ministry has received your application, and the second cheque will arrive approximately six months later.

If you are a resident in a long-term care facility, you can have your payment(s) sent directly to the facility. You or your agent must indicate the name and address of the facility in the section "Payment to be made to person and/or address other than above".

Notification of Changes

It is your responsibility to notify both ADP and OHIP whenever you change your address or surname. This is extremely important. If you lost your health card and now have a new one, ADP needs to be notified. ADP automatically checks your health card number and date of birth with the health card database. Other Details

RECEIPTS • You must keep copies of the receipts for your ostomy supplies for two years. The Ministry of Health and Long-Term Care may ask you to produce receipts to confirm your claim. For income tax purposes, you can only claim receipts for expenditures that exceed the amount of your grant.

RE-APPLICATION • You do not need to re-apply each year to receive your grant. Once your application has been approved, ADP will automatically send you a payment every six months. ADP confirms with the Ontario Health Insurance Plan (OHIP) that you are still a resident of Ontario and that your health card number is still valid.

PURCHASING SUPPLIES • You are not restricted to purchasing your supplies from a particular supplier or manufacturer. You can buy your ostomy supplies from any store in Ontario that sells these products. Remember, keep those receipts!

INSURANCE • Some private insurance plans will cover additional expenses concerning your ostomy supplies. If you have private medical coverage, check with your insurer or agent to see if they will pay for your additional costs.  [Top]
