ADP Grant Survey and Poll

Cause

The Ontario Ministry of Health and Long-Term Care Assistive Devices Program (ADP) grant has not been increased in 11 years. LDOA would like to know what you think about this – is it time for change or let sleeping dogs lie? Cast your vote, leave your comments, take our survey, and let's see where we stand on this issue.

If you haven't already done so, you may find it helpful to read the following two articles before completing the survey and/or leaving any comments.

Article #1 • For information regarding the ADP grant  (also referred to as supplement) provided by the Ontario Ministry of Health and Long-Term Care for persons with permanent ostomies, refer to LDOA's article, Ostomy Supplies Grant.

Article #2 • For tips on how to stretch your ADP cheque, refer to UOAC's article, ADP and the Ostomate.

Effect

  • Completing the survey is done anonymously.  Furthermore, you are not required to leave your name and email address in the comments section found in the Rate the ADP Poll. If you choose not to leave your name and email address, the word "Anonymous"  will appear beside your comment.

  • Anyone with a permanent or temporary ostomy or who is responsible for purchasing the ostomy supplies of someone with an ostomy may complete the survey. As this issue deals with the Assistive Devices Program provided by the Ontario Ministry of Health and Long-Term Care, we please ask that only Ontario residents complete the survey and voting poll.

  • You may only complete the survey and poll once each. Thereafter, you will be blocked from voting but you can view the results and read the comments as often as you wish.

  • The survey and poll will remain open until such time as LDOA feels it has enough responses to determine if persons with ostomies are satisfied with the status quo of the ADP grant or would like to take some action to increase the annual amount.

If you know someone with an ostomy who resides in Ontario, please do not hesitate to recommend this page. If you are a Webmaster of an ostomy web site in Ontario, we would be grateful if you would provide a link to this page on your web site. The direct link is  http://www.ldoa.net/adp_survey_intro.htm.

The more responses we receive, the better we can determine where persons with ostomies stand on this issue. Thank you.
